LGS Recreation Programs

LGS Recreation programs offer life-long education to help you continue your personal growth and development. Programs of all kinds help those in the 55 plus age group move into new areas of enrichment and fulfillment.

These include Case Management. Case Management is a collaborative process of assessment, planning, and advocacy. The Case Manger will meet with you and your family, at no cost to you, to discuss problems, offer recommendations, and if appropriate, make referrals to social service agencies. The Case Manager is also available to help complete complicated applications required to receive social service assistance. It is the goal of Case Management to provide help and advice to seniors and to help them age in place as appropriate.

Available in 170+ languages 24 hours a day, Santa Clara County 211 is a United Way of Silicon Valley free, confidential, multi-lingual referral service available to all residents of Santa Clara County.

Available in English from 8 am to 5 pm, Monday through Friday, Sourcewise provides information on coordinating services and support systems for all adults, including seniors, their families, and caregivers, in Santa Clara County.

These programs and services are available to Los Gatos seniors and address a range of needs from care management to day care.

El Camino Hospital

El Camino Hospital is a nonprofit hospital group in Los Gatos and Mountain View. They provide an array of inpatient services, a full service emergency room, and various outpatient services. El Camino also operates Pathways, a nonprofit home healthcare and hospital organization.

Good Samaritan Hospital

Good Samaritan Hospital is a 474-bed acute care hospital and a recognized leader in neurosciences, comprehensive cancer services, advanced cardiology and innovative surgery techniques. Good Samaritan is a certified stroke hospital and an accredited Chest Pain Center.

Palo Alto Medical Foundation

Palo Alto Medical Foundation's Los Gatos Center offers primary care for children and adults, Urgent Care, diagnostic imaging and a full lab. The 40,000-square-foot Los Gatos Center has more than 50 exam and procedure rooms and is home to more than 30 physicians.

Housing Conservation

The Town offers no-and low-interest loans to income-eligible Los Gatos homeowners to finance home rehabilitation projects. Please call the Santa Clara County Office of Affordable Housing, which administers the program for Los Gatos.

Live Oak Adult Day Services - Adult Day Care

As we age, getting together with friends becomes even more important. This program provides age-appropriate social and recreational activities, meals, and attention from professional staff. It also provides respite for participants' regular care givers, usually family members. Information relating to elder care, counseling support, and referral to other relevant community resources are also provided for the family members of program participants.

Live Oak Senior Nutrition Center

Provides not only a good lunchtime meal, but a chance to get together with friends, and to make some new ones! This program provides a lunch meal each Monday through Friday. Health education programs, health screening, and educational programs are provided on-site. Live Oak also provides outreach services for home bound clients, including safety checks, friendly visits, service referrals, and volunteer service projects.

Long-Term Care Ombudsman

If you have concerns about the quality of care being received by yourself, a loved one, or a friend in a long term care facility, the 
Ombudsman Program is here for you. This service provides volunteer ombudsmen to receive, investigate, and resolve complaints made by, or on behalf of, long term care facility residents. It provides for regular site visits by staff and certified volunteers to facilities where face-to-face contact is made with individuals at risk for isolation, exploitation, personal rights violation, and abuse. Operation Brown Bag

Second Harvest Food Bank, bags of nutritionally balanced food are distributed and delivered to seniors every Friday at 9:00 a.m. in the Large Hall of the Los Gatos Adult Recreation Center. Participants must be low income seniors and must submit an application. This program also needs volunteers to help stuff the food bags. Call Operation Brown Bag at (408) 266-8866.

When you live by yourself, it's important to know that someone cares how you are each day. The Los Gatos-Monte Sereno Police Department provides a free daily "check-in" telephone call for registered seniors.

Parents Helping Parents

This agency provides information, training, and support services to parents, grandparents, and adult caregivers of children with special needs. Services include peer counseling, support groups, information and referral, gifts for newborn infants, multilingual outreach, early intervention services, and integrated playgroups. The Health Place

San Jose State University's Nursing Program can help you with health concerns right in your own home. This service provides in-home nursing services for the frail elderly, including blood pressure screening and monitoring. They provide consultation for agencies, clients, and caregivers, as well as education, and information and referral.
